Christian Health (CHCC) has been fostering health, healing, and wellness to people in North Jersey for more than a century. We offer the most complete continuum of senior-life, short-term rehab, and mental-health services available in the area. Our commitment to caring begins with recognizing the intrinsic value of all individuals and helping them achieve their highest potential for physical, emotional, and spiritual well-being.

Guiding our mission is Person- and Family-centered Care (PFCC), a philosophy focusing on treating patients, residents, clients, consumers – and their family members – as partners on the care team. PFCC is about responding to their individual needs in all that we do,  giving them the ability to make choices about care and daily activities, and understanding their perceptions of care – what is important to them, how well we deliver care, and, more important, how can we analyze and change care practices to improve outcomes. At CHCC, PFCC involves four components: respect and dignity, information sharing, participation in care and decision-making, and collaboration on policy and program development, implementation, and evaluation.

In order for PFCC to be successful, we need to hear the voices of those we care for and those who love them. Members of our Patient and Family Advisory Council provide those voices. They partner with us to improve our care by sharing their stories, providing feedback, participating in discussion groups, and working on short-term projects. We are enormously grateful for their participation and input.

PFCC is not unique to Christian Health. It’s a practice in place in small community hospitals in rural Midwest towns to sprawling medical centers in bustling West Coast cities. But what is unique here, at Christian Health, is you, your family members, and your neighbors.
